Although all the 9/11 stuff was bullshit, it’s confirmed that there was meant to be a chapter 3 (Peace), and presumably more proper missions and fleshed out story in chapter 2. It’s pretty clear, whatever the reason was, that the game is no where near complete and Konami forced its early release, presumably when the whole schism with Kojima happened.
If a game being released almost 3 full years after first being shown is considered “early” I might sympathize with Konami a little.
I think what they were going for with the medic thing is that “YOU were the medic all along and you abandoned your life to be Big Boss because this game” cause your name is on the postcards, which is probably even a way worse twist/asspull than assuming the medic was alright with everything that happened with him.
